Transfer Chart has been contracted by the physician, hospital, physician practice or other entities from where you are requesting medical records. Transfer chart follows all Federal and State HIPAA regulations. Transfer Chart follows all fee schedules set by state guide lines.

Our procedure when requesting medical records:

The medical records authorization needs to be completely filled out stating what is to be copied, signed and dated by the patient if over 18 years old or by legal guardian if under 18 years old.
Once the medical records authorization is filled out completely, forward the request to Transfer Chart for processing. Your request can be sent either by:

Once we have received the medical authorization we will process your request. We will send a prepayment invoice to you either by fax, email, or mail. There is a charge for medical records unless records are sent directly to another doctor.
Payment can be made online, by credit card, or by check. Once we have received payment, records will be sent out immediately. We can send medical records by fax, mail, or secure email.
